Purpose & Scope Of Privacy Policy

Screen2Work simplifies the process for users to check their current health condition to identify if they are experiencing COVID-19 symptoms based on Centers for Disease Control and Prevention guidance. This is achieved through responding to a questionnaire. Users that report actual or potential COVID-19 symptoms will be notified and provided additional information. Inception Health, Inc., a wholly owned subsidiary of Froedtert Health, Inc. (“we,” “our,” or “us”) respect the privacy of your personal information, so we have prepared this Privacy Policy to assist you in understanding the personal information that is collected when you use Screen2Work, how that information is used and shared, and how it is safeguarded.

We are closely monitoring COVID-19 and related local, state, and federal mandates and guidance, which remains a fluid situation. As a result, from time to time we may change this Privacy Policy. If we do, we will post an amended version within the mobile application. Please review this Privacy Policy periodically. What Personal Information Does Screen2work Collect?

Through your use of Screen2Work, we may collect information about you and from which you can be identified (“personal information”), including your email address, certain COVID-19 health-related information in the questionnaire of “yes” / “no” responses, the timestamp from when you complete the screening and the score generated based on your responses.

Screen2Work is not a contact tracing technology. Therefore, it does not use wi-fi, geolocation or Bluetooth technology for contact tracing purposes.

How Do We Use Your Personal Information?

We use your personal information to verify your account login, to provide support, to communicate with you about Screen2Work or to inform you of any changes to Screen2Work, to maintain and improve Screen2Work, to conduct research and analysis on app usage and trends (e.g., symptomatic surges), to protect our legal rights and the rights of others, to efficiently maintain F&MCW operations, and to comply with applicable law. Your employer may use such personal information for their own lawful purposes.

Who Do We Share Your Personal Information With?

We may share personal information collected through Screen2Work with your employer or as directed by your employer. Additionally, we may share personal information with third parties if required to do so by law or in connection with any current or prospective legal proceedings; to establish, exercise, or defend our legal rights, including providing information to others for the purposes of fraud prevention; to any person who we reasonably believe may apply to a court or other competent authority for disclosure of that information where, in our reasonable opinion, such court or authority would be reasonably likely to order its disclosure; to any other person or entity as part of any business or asset sale; and to any other person or entity where you consent to the disclosure.

Except for the limited circumstances listed, personal information collected through Screen2Work will not be shared with individuals or entities outside of Inception Health, its affiliates, and subsidiaries.

Retaining Your Personal Information

We will only keep your personal information for as long as necessary to fulfill the purposes we collected it for, including for the purposes of satisfying any legal or reporting requirements, or for a legitimate purpose.

Security

We maintain commercially reasonable security measures to protect the personal information we collect and store from loss, misuse, destruction, or unauthorized access. However, no security measure or modality of data transmission over the Internet is 100% secure. Although we strive to use commercially acceptable means to protect your personal information, we cannot guarantee absolute security.

Children’s Information

Screen2Work is not intended for children under 13 years of age. We do not knowingly collect, use, or disclose personal information from children under 13.
