Family (Proxy) Access Information
Who can enroll?
MyChart offers patients access to portions of their Froedtert & the Medical College of Wisconsin medical record over a secure Internet connection. MyChart is unavailable at clinics in which the electronic medical record is not in use.
For specific questions on MyChart availability, please contact the clinic where you receive care.
How do I enroll?
If you are new to MyChart, get started by clicking or tapping Create New Account.
How do I request access (known as proxy access) to a family member in MyChart?
Family members and other caregivers can be granted access to another person’s MyChart account. In order to be a Proxy for another adult, child, or adolescent, you must have an active MyChart account. If you do not have a Froedtert & MCW MyChart account, first, you must click Create New Account.
You have two options to proceed.
Option 1: Submit your request electronically via MyChart
Log in to your MyChart Account on the web (not available on our mobile experience).
Go to Your Menu > Account Settings section > Personalize
At the bottom of the web page, identify the proxy access you are requesting and click the link to submit your request electronically.
Option 2: Submit by printing and completing a paper form
Identify from the list below what identify the proxy access you are requesting and click the link to print the appropriate form.

- Child Under 12 Proxy Request Form (PDF)
If you choose to download/print the form(s), when complete you may choose from the following options:
• E-mail to healthinformation@froedtert.com
• OR return to any Froedtert & the Medical College of Wisconsin clinic location
• OR fax to the Health Information Management Department at 414-259-1244
Proxy access for patients age 12-17 cannot be submitted electronically you must use the form below and it must be printed.
When complete you may choose from the following options:
